Glenn Charlton

Glen currently serves as a physician consultant to Primary Health Network (PHN), a large Federally Qualified Health Center throughout western and central Pennsylvania.  Medical training occurred at Jefferson Medical College in Philadelphia, residencies at Washington Hospital (Family Medicine) and in Cincinnati (Occupational Health).  Clinical experience consisted of more than 35 years in Emergency Medicine and various administrative positions. A few years ago, while serving as Chief Medical Officer at PHN, he was involved in the implementation of new Medicare Annual Wellness Visits at several PHN sites.  More recently, as a semi-retired physician consultant, Glenn has returned to PHN to assist in the further expansion of Annual Wellness Visit services throughout the network as well as initiating Chronic Care Management services.  He has been involved in the revision of documentation forms in the Electronic Health Record to make visits more efficient and to assist in the acquisition of data relevant to the GWEP-PSU grant.
